Your designated beneficiaries will receive a lump-sum payment should you pass away while employed with us.<\/li>\n<li><strong>Accidental Death and Dismemberment Insurance<\/strong> coverage equal to your life insurance amount. Your designated beneficiaries will receive an additional full or prorated lump sum payment should you lose a limb or pass away as the result of a covered accident.<\/li>\n<\/ul>\n<p>This benefit reduces to 50% at age 65 and terminates at age 75 or earlier retirement.<\/p>\n<h2>Short Term Disability (STD) Highlights<\/h2>\n<p>Your weekly STD benefits may replace up to 75% of your salary, to a maximum benefit of $1,750 per week and lasts up to 16 weeks.<\/p>\n<p>After this period, if you are still disabled and unable to work, you may become eligible for Confluent\u2019s long-term disability (LTD) benefit (see below).<\/p>\n<h3>When do payments begin?<\/h3>\n<ul>\n<li>If you become totally disabled because of an accident, you are eligible for STD payments on the day you become disabled or the first day you consult a doctor (whichever is later).<\/li>\n<li>If you become disabled because of an illness, you are eligible for STD payments after 7 days of disability or the first day you consult a doctor (whichever is later).<\/li>\n<\/ul>\n<p>STD payments are a taxable benefit.<\/p>\n<h2>Long Term Disability (LTD) Highlights<\/h2>\n<p>If your STD benefit ends and you\u2019re still unable to work, LTD benefits provided to you through Confluent may replace up to 75% of your salary for the length of your qualified disability, up to a maximum benefit of $15,000 per month (or $9,500 without evidence of good health).<\/p>\n<p>The LTD benefit is offset by other benefits received, such as government disability assistance and Confluent leave pay policies, as applicable.<\/p>\n<p>The plan provides a qualifying disability period of 16 weeks, with taxable benefits payable until age 65.<\/p>\n<p>Please note: In order to receive full LTD coverage, you may need to complete and send a Health Statement to Sun Life at <a href=\"mailto:medicaluw@sunlife.com\">medicaluw@sunlife.com<\/a>.
