Your Collective Health Benefits Assistant

Collective Health is your one-stop for all your core benefit needs. This service is here to make healthcare easier for all Confluent employees enrolled in Collective Health plans. You can get details on your benefits, help navigating through them, and connect to resources to help with your benefits needs as well as claims and advocacy support, ID card issuance, and coordination of benefits—all through Collective Health.

 

All employees can get access to pharmacists, social workers, dietitians, nurses, and care coordinators through Collective Health at no cost to you who can help you:

  • Support you in setting goals to improve your quality of life
  • Assemble a customized care plan to reach your goals
  • Support you each step of the way in achieving your goals

  • Managing infusions, injectables, or multiple medications
  • Pregnancy, newborn, or birth-related complications
  • Managing surgeries and traumatic injuries
  • Genetic and rare conditions
  • Gender affirmation surgery and services
  • Developmental conditions
  • Neurologic, cardiac, or gastrointestinal conditions
  • Cancer, strokes, asthma, diabetes
  • Navigating disabilities
  • Assistance connecting you with community resources

Additional support is available beyond what is listed above. You are encouraged to call even if your issue is not listed. Reach out by calling 833-834-1170 (Monday – Friday, 9am – 6pm PST) or send a secure message from your Collective Health account.

If you are enrolled in a Collective Health plan (Anthem, Delta Dental, and/or VSP) you can call Collective Health Member Advocacy to get respectful, inclusive support for your benefits. Your advocate can help:

  • Answer questions about how screenings or other services are covered
  • Help navigate potential billing and claims disputes
  • Guide members through obtaining any necessary pre-authorizations
  • Utilize the Get Care tool to search for a provider that meets your needs by specialty, location, name, and language
  • Check to ensure providers are within your plan
